An application like this will be easily turned down. Please explain your reasoning for each of the statement thorough.
Example:

Professions (with Skill Level): Mine/JC, I picked up jewelcrafting because of the gem bonus with Dragon's eyes and read that JC is one of the most versatile raiding profession. I currently have mining as my secondary profession because it was due to help me level up my jewelcrafting. I am able to change my profession to adjust to the raiding schedule.

Another example is:

Raid Addons Installed: Yes i currently have the most updated Omen, DBM, Healbot, Decursive from Curse.com.
